Our Concierge Care Model

PBHA operates as a private, concierge-style practice designed for a more continuous and thoughtful therapeutic process. This model supports clearer thinking, more intentional decisions, and sustained reflection over time.

Tailored Matching

Matches are based on personalized fit, not availability.

Ongoing Relationship

Care is approached as an ongoing professional relationship rather than isolated sessions.

Discretion is Central

Personal, relational, and professional experiences can be explored with a high degree of privacy and care.

People often come to therapy not because something is broken, but because something no longer feels fully understood.

Life may be functioning on the surface—responsibilities are met, momentum continues—but internally, patterns or decisions feel unclear.

At PBHA, therapy is a process of reflection, integration, and clarity.

Rather than focusing on symptoms alone, the work centers on understanding how you think, relate, and make decisions—shaped by expectations, roles, and past experience.

Over time, this leads to clearer thinking, more grounded decisions, and a stronger sense of direction.

PBHA works with individuals whose lives carry complexity, responsibility, or visibility.

Many operate in demanding environments. Others come from backgrounds shaped by expectation, resources, or layered family dynamics.

What connects them is not a role, but a shared way of moving through the world:
A tendency to think deeply, carry responsibility internally, and approach challenges through analysis, effort, or control—often with little space to step back.

At PBHA, the focus is not simply on problems, but on how experiences are understood and navigated over time.

Clients often come to explore:

  • A collaborative process grounded in insight and reflection
  • Thoughtful exploration rather than prescriptive solutions
  • Greater clarity in how you think, relate, and decide
  • A stronger sense of agency in how you move forward

Therapy at PBHA is relational, exploratory, and intellectually engaged.

Sessions are guided by conversation, focusing on patterns, emotional responses, and the logic shaping behavior and decisions.

The Process is Collaborative

Together, we explore the experiences shaping how you understand yourself and the world—bringing clarity to patterns that may have felt automatic.

Progress you can see

Change tends to last when people can see it happening. PBHA therapists check in with clients on progress at regular points in the work, so there is a shared sense of what is shifting and what still needs attention. Every clinician trains within the same model and receives ongoing supervision and professional development, so care is consistent across the practice. The relationship also does not have to end when regular sessions do: clients are invited to optional check-ins at three months, six months, and one year after completing therapy.

Curiosity is central

As insight develops, clients are better able to recognize their choices and decide how they want to engage with their lives and relationships.

This Model provides:

  • A collaborative process, with goals shaped together from the first session
  • Thoughtful exploration, grounded in shared reflection on progress over time
  • Room to reflect openly on how the work, and the fit, are serving you
  • Greater clarity in how you think, relate, and decide, with change you can recognize
  • A stronger sense of agency, and continuity that extends beyond the final session

Our services

Founders

PBHA was founded by Sean Orpen, Danielle Ruthfield, and Dr. Ethan Schwab, whose combined experience spans more than fifty years across clinical, academic, and professional settings.

Their work includes community mental health, universities, research, private practice, and consultation with major organizations. Alongside this, they have built and led businesses while navigating the realities of family and professional life.

These experiences shape how they understand people.

Each brings a distinct, thoughtful perspective grounded in relational work, psychological depth, and real-world experience.

PBHA was created as an intentional alternative to traditional group practice—designed to prioritize depth, personalization, and meaningful therapeutic work.
