PBHA Fees, Insurance & FAQ

What PBHA costs, how insurance and reimbursement work, and how the process runs from first contact to your first session.

A session at Premier Behavioral Health Associates (PBHA) costs $275 per clinical hour. PBHA is out-of-network and does not bill insurance directly, but provides a superbill you can submit for possible out-of-network reimbursement. Getting started takes four steps: you reach out, an intake specialist matches you with a therapist, your first session is scheduled, and you meet to confirm the fit is right.

The sections below cover fees and billing first, then the fuller how-it-works walkthrough and who PBHA tends to see.

Fees, Insurance & Billing

Sessions are $275 per clinical hour. A clinical hour is 50 minutes with your clinician, billed as a full hour. The remaining time supports note completion and continuity between sessions. Extended sessions are billed proportionally at the same hourly rate:

Session Length Cost
50-minute clinical hour (standard) $275
90 minutes $412.50
2 hours $550

Couples sessions are billed at the same $275 hourly rate as individual sessions. Fees for family sessions and therapy intensives may differ from the standard rate; your clinician can confirm the applicable fee during your consultation.

PBHA is an out-of-network provider and does not bill insurance directly or hold contracts with insurance companies. This is a deliberate choice, keeping the clinical relationship, not a third party’s coverage rules, in control of session length, frequency, and treatment approach. Many clients with out-of-network mental health benefits are still able to receive partial reimbursement; we recommend contacting your insurance provider directly to understand what your specific plan covers.

Yes. A superbill is a detailed invoice, including session dates, fees paid, and the relevant procedure codes, that you can submit to your insurance company to seek out-of-network reimbursement. Superbills are available upon request; simply let your clinician or our administrative team know, and one will be prepared for you.

Your superbill will reflect the CPT code corresponding to your session:

Service CPT Code
Initial diagnostic evaluation 90791
45-minute individual psychotherapy 90834
60-minute individual psychotherapy 90837
Couples or family psychotherapy (conjoint) 90847

Couples and family sessions are also typically paired with ICD-10 diagnosis code Z63.0 (problems in relationship with spouse or partner), a code for relationship distress rather than an individual mental health diagnosis. Many insurance plans reimburse this kind of code less consistently than an individual diagnosis, which is worth confirming with your insurer specifically if you’re seeking reimbursement for couples or family work.
Your insurance company can tell you what portion, if any, of these codes are reimbursable under your specific plan. We recommend asking about your out-of-network mental health benefits before your first appointment if reimbursement is a consideration for you.

A Good Faith Estimate is a written estimate of expected costs, provided before treatment begins. Under the federal No Surprises Act, clients who are not using insurance to pay for their care have the right to receive one. The estimate covers the cost of the sessions discussed at the time of scheduling. If your actual bill is $400 or more above your Good Faith Estimate for the same provider, you have the right to dispute the charge. You can request a Good Faith Estimate at any time, when scheduling or before you schedule at all. PBHA will provide one in writing within the timeframe required by law. Keep a copy of your estimate; you’ll need it if you ever compare it against a bill.

The No Surprises Act is a federal law that took effect in 2022, requiring healthcare providers to give clients clear, upfront information about expected costs. For questions about your right to a Good Faith Estimate, you can also visit www.cms.gov/nosurprises, email FederalPPDRQuestions@cms.hhs.gov, or call 1-800-985-3059.

We ask for at least 48 hours’ notice to cancel or reschedule a session. Sessions cancelled or missed with less than 48 hours’ notice are charged the full session rate automatically. This policy exists because each session holds a specific place in your clinician’s schedule, one that could otherwise be offered to another client.

We accept Visa, Mastercard, and both FSA and HSA cards. Payment is due at the time of your session.

How It Works

PBHA works with executives, physicians, founders, attorneys, investors, and others whose professional lives carry significant complexity, responsibility, or visibility. What tends to bring someone to PBHA isn’t usually a single presenting issue so much as a pattern: the sense that success and interior experience have drifted apart, gradually and without a clear point where that started, that the tools that once managed pressure have stopped working as well as they used to, or that a relationship, a transition, or a private part of life has been running on reduced attention for longer than anyone noticed. The common thread isn’t a diagnosis. It’s a life complicated enough that generic care rarely fits it well.

PBHA clinicians work across a range of concerns common to this population, including:

  • Executive stress and burnout
  • Anxiety and perfectionism
  • Relationship and couples work
  • Sex therapy and intimacy
  • High-functioning depression
  • Life transitions and identity
  • Family wealth and parenting
  • Therapy intensives, for concentrated, accelerated work

Not every clinician works in every area. Part of what an intake specialist does is match you with a therapist whose specialty actually fits what you’re bringing to the work, rather than whoever happens to have an open slot.

Getting started at PBHA follows a simple sequence, built to make sure the fit is right before anything is decided.

  1. Reach out. You contact PBHA to begin the process.
  2. Speak with an intake specialist. An intake specialist will talk through what you’re looking for and match you with a therapist based on your preferences, clinical needs, and scheduling availability.
  3. Book your first session. Once you’re matched, your first session is scheduled and you’re added into PBHA’s system.
  4. Have your first session. Both you and your therapist use this session to confirm the match is right before deciding to move forward together.

Most people, once matched, are ready to begin. Still, walking into a first clinical session with someone you haven’t yet spoken to isn’t for everyone. If you’d like more certainty beforehand, you can request a brief consultation call with your assigned therapist to get a sense of each other before the clinical work begins. This option is available to anyone who wants it, though it’s neither required nor a substitute for the intake matching process, simply there for people who want a bit more before they start.

PBHA sees clients through telehealth, which allows for the flexibility and discretion this work often calls for. You’ll need to be physically located in Washington State at the time of your session, or in another state where your specific clinician holds an active license. Because PBHA clinicians are licensed individually rather than as a single group, the states available to you may vary depending on who you work with. Your intake specialist can confirm this before anything is scheduled.

Telehealth at PBHA is simply the format the work happens in, not a scaled-down version of therapy. The strength of the relationship, the clinical rigor, and the depth of the conversation depend on the therapist and the work itself, not on whether two people share a room. For the population PBHA tends to serve, telehealth solves a real practical problem: fitting consistent care into a schedule that rarely has room for a commute or a waiting room. Clients can meet reliably through travel, relocation, or a demanding week in a way in-person care often can’t accommodate.

Privacy at PBHA is built into how the practice operates, rather than added on as a feature. Sessions take place over SimplePractice, PBHA’s HIPAA-compliant EMR and video platform, with no shared waiting room and no physical office where you might be seen coming or going. Clinical records are protected under HIPAA and are never shared without written authorization, except where required by law. Because PBHA does not bill insurance, your care also never depends on a diagnosis being placed on file with a third-party payer, one less place your information exists and one less thing to manage. For clients whose professional lives carry real visibility, that level of discretion is often part of what makes starting the work possible in the first place.

We ask for at least 48 hours’ notice to cancel or reschedule a session. Sessions cancelled or missed with less than 48 hours’ notice are charged the full session rate automatically.
